Just when the doctors think they found a cure to the plague, it relapses. Chae Sun contracts the plague. Yoo Do Ji returns from China. The police general reaffirms that he will arrest Hur Jun after a cure for the plague is found.

In recognition for his work on finding a cure for the plague, Hur Jun gets promoted to 6th rank. Hur Jun's rivals catch wind of his tainted past and inform their superiors.

Hur Jun and Da Hee are arrested. The King demands Hur Jun to be exonerated of all his crimes. When the Queen falls ill with a myocardial infarction, the King sends for Hur Jun hoping he can revive her.

Yoo Do Ji's scheme to get Hur Jun kicked out of the palace backfires as Hur Jun is promoted to 3rd rank. Shortly thereafter, the Japanese invade. Hur Jun attempts to salvage the medical books from the library before it's destroyed.

A mob sets the palace and hospital alight. Hur Jun rushes into the burning building after hearing Lady Ye Jin is still inside. The King and his entourage flee Seoul as the Japanese invasion closes in. Hur Jun gets separated from his family

Sang Hwa sacrifices himself. Yoo Wol gives birth. The King's advisers urge him to escape to China. Even though Hur Jun successfully makes it to Pyongyang castle, the King feels betrayed. Yoo Do Ji looks after the King's health.

At Pyongyang castle, the Prince fights back the Japanese army. In Weeju, Inbin Queen's son faints. Hur Jun is ordered to go to Weeju to look after the gravely ill King. Gyum finally understands his father. Yoo Do Ji is thrown into prison.

Hur Jun presents Yoo Do Ji with an opportunity to save his life. The two doctors reconcile. Hur Jun begins to write a medical book for commoners. Lady Ye Jin is forced to leave the palace. Hur Jun is promoted to first class official.

Ten years later, Inbin Queen gives birth to a son. Both political factors attempt to court Hur Jun. The palace doctors struggle to compile the finishing touches to their medical textbook. The prince is diagnosed with malaria.

Both political factions are confused after Hur Jun prescribes the prince with arsenic as medicine for his malaria. Hur Jun gets concerned that the doctors will not be able to complete the textbook. The King falls severely ill.

As the King's death imminently approaches, some court officials try to persuade Hur Jun to keep silent about King's written declaration to enthrone Prince Gwanghae as king. Hur Jun is exiled.

Even in exile, countless patients go to Hur Jun to seek medical care. The medical text Dongui Bogam is completed. Hur Jun retires to the old hospital in Sanum to treat poor patients, eventually becoming sick himself.
